Purpose: To serve individuals who are homeless through a continuum of care, emergency shelter, intake and assessment, transitional housing and support services, with the goal of gaining or regaining economic self-sufficiency. This program serves the entire county. Eligibility: Homeless individuals are served based on HUD's definition. The Homeless Services Program has three (3) Homeless Outreach Teams. Each team consists of employees from Division of Human Services, CARP, and Oakwood. Assessments are done together and individually. These teams assess by appointment as well as regular outreach into the community. Individuals must be able to work, ready to remain sober, and/or compliant with mental health recommendations. The HOT Team will conduct assessments at the Senator Philip D Lewis Center. You must request an assessment referral number by going to the Navigation Center at The Lord’s Place Café Joshua, 2808 Australian Ave, West Palm Beach. The telephone number is (561)494-0125. The Homeless Outreach Team will be traveling to various locations in Palm Beach County Monday through Friday, offering services and conducting on site assessments. |
