Forms are available and various other information regarding the procedures one must follow when making requests in the Family Courts. Walk-ins taken (as time permits) Monday, Tuesday and Wednesday, no appointment necessary. Cost $10. Thursday and Friday, appointments may be required. Scheduled appointments cost $25.

By State Law, the Commission on Ethics in Tallahassee each year must compile a list, by county, of all local government officials required to file a Financial Disclosure Statement. The list is sent to the Supervisor of Elections, who sends a Financial Disclosure form to each official on the list. The list consists of all elected municipal officials, elected County officials, and members of various special districts and advisory boards that the Commission on Ethics has ruled must file. The disclosure forms must be filed with the Supervisor of Elections no later than July 1, of each year. Contact the Supervisor of Elections for more information.
