Fraud
Fraud A government employee was soliciting $1,000 from homeowners as a bribe to close out county code violations. As a result of receiving the bribe, the government employee subsequently falsified county records to conceal several code violations.
Waste
Waste A city wasted $145,000 in attorney fees defending a citizen's lawsuit to obtain public records when the records could and should have been provided to the citizen.
Misconduct  
Misconduct Two government employees circumvented the procurement process to purchase equipment from a favored company rather than going out for competitive bid. The specific equipment ordered and purchased should have been advertised for competitive bid.
Mismanagement
Mismanagement A vendor was transferring monies between restitution accounts in order to meet operational expenses. The government entity was aware of this issue in addition to having knowledge that the vendor's independent auditor report expressed an on-going concern as to the vendor's ability to continue operations. The government entity had no documented procedures in place to perform contract oversight to ensure contract deliverables/services were met. Although the vendor submitted reports as required by the contract, the government entity did not perform any validation to determine if the reported data was accurate.
Abuse  
Abuse

A government employee induces a "vehicle for hire" company to come to the Palm Beach International Airport to illegally pick-up a passenger; represented himself as a law enforcement officer by detaining the company employee; and illegally searching the vehicle.
